MetLife Eligibility: Dental, Life, STD, LTD, and Vision

MetLife Dental: In order for a group with 2-4 eligible to be considered eligible for MetLife Dental, the group must have 100% participation and the employer must contribute 100%. In order for a group with 5+ eligible to be considered eligible for MetLife Dental, the group must have at least 50% participation with a minimum of 5 enrolled and the employer must contribute at least 50%.

Please note, for a group to be considered eligible for MetLife Voluntary Dental, the group must have at least 35% participation with a minimum of 5 enrolled and the employer must contribute at least 50%.

MetLife Life: In order for a group to be considered eligible for MetLife ­Non-Contributory Life, the group must have a minimum of 5 enrolled with 100% participation. In order for a group to be considered eligible for MetLife ­Contributory Life, the group must have 10+ eligible, 5+ enrolled, and a minimum of 50% participation. In order for a group to be considered eligible for MetLife Voluntary Life, the group must have 10+ eligible, 5+ enrolled, and a minimum of 25% participation.

MetLife STD: In order for a group to be considered eligible for MetLife ­Non-Contributory STD, the group must have a minimum of 5 enrolled with 100% participation. In order for a group to be considered eligible for MetLife ­Contributory STD, the group must have 10+ eligible, 5+ enrolled, and a minimum of 50% participation. In order for a group to be considered eligible for MetLife Voluntary STD, the group must have 10+ eligible, 5+ enrolled, and a minimum of 25% participation.

MetLife LTD: In order for a group to be considered eligible for MetLife ­Non-Contributory LTD, the group must have a minimum of 5 enrolled with 100% participation. In order for a group to be considered eligible for MetLife ­Contributory LTD, the group must have 10+ eligible, 5+ enrolled, and a minimum of 50% participation. In order for a group to be considered eligible for MetLife Voluntary LTD, the group must have 10+ eligible, 5+ enrolled, and a minimum of 25% participation.

MetLife Vision: In order for a group to be considered eligible for MetLife Vision, the group must have at least 10% participation with a minimum of 5 employees enrolled.
