Humana Dental: In order for a group to be considered eligible for Humana Dental, the group must have at least 50% participation with a minimum of 2 employees enrolled. There is not an employer contribution minimum.
Humana Life: In order for a group to be considered eligible for Humana Life, the group must have a minimum of 2 employees enrolled and also at least 50% participation. Please note, for a group to be considered eligible for Humana Voluntary Life benefits, the group must have a minimum of 5 employees enrolled and it must be bundled with either Medical or Dental.
Humana Vision: In order for a group with 2-4 eligible to be considered eligible for Humana Vision the group must have 50% participation with a minimum of 2 employees enrolled, and must be bundled with either Medical or Dental. In order for a group with 5+ eligible to be considered eligible for Humana Vision, the group must have at least 50% participation with a minimum of 5 employees enrolled.
