BCBSIL Dental: In order for a group to be considered eligible for BCBSIL Contributory Dental , the group must have at least 70% participation and the employer must contribute at least 50%. In order for a group to be considered eligible for BCBSIL Voluntary Dental, the group must have at least 25% participation, and there is no employer contribution minimum.
BCBSIL Life: In order for a group with 1-4 eligible to be considered eligible for BCBSIL Life, the group must have 100% participation and the employer must contribute 100%. In order for a group with 5+ eligible to be considered eligible for BCBSIL Life, the group must have at least 75% participation and the employer must contribute at least 25%.
Please note, for a group to be considered eligible for BCBSIL Voluntary Life benefits, the group must have at least 15% participation and a minimum of 10 enrolled.
BCBSIL STD: In order for a group with 1-4 eligible to be considered eligible for BCBSIL STD, the group must have 100% participation and the employer must contribute 100%. In order for a group with 5+ eligible to be considered eligible for BCBSIL STD, the group must have at least 75% participation and the employer must contribute at least 25%. Please also note, for a group to be able to elect STD, they must also elect Life.

In order for a group with 5+ eligible to be considered eligible for BCBSIL LTD, the group must have at least 75% participation and the employer must contribute at least 25%. Please note, for a group to be considered eligible for BCBSIL Voluntary LTD benefits, the group must have at least 15% participation and a minimum of 10 enrolled. Please also note, for a group to be able to elect LTD, they must also elect Life.
BCBSIL Vision: In order for a group to be considered eligible for BCBSIL Vision, the group must have at least 10 employees enrolled. The group must also have at least 75% participation and at least 80% employer contribution. If employer contribution is 100%, then participation must be 100%. Please note, BCBSIL Voluntary Vision does not have an employer contribution minimum.
