In order to request a G&A Partners quote, we would need to collect the following information from the group:

• A completed version of the G&A Partners group and personal health questionnaires
• Completed G&A Partners quoting census
• A copy of the group’s current medical invoice, renewal, and health SBCs
• Worker’s Comp Declaration page and 3 years of currently-valued loss runs
• Confirmation of the group’s current state unemployment rate(s)
