Allied National Enrollment Material Includes:
- Copy of binder check & mailed physical binder check
- Completed and signed version of Allied National specific forms
- Most recently filed quarterly wage & tax + payroll for any employee not listed
- Health Plan enrollment/waiver form for all full-time employees
- Copy of the group’s most recent prior carrier bill and renewal letter
How Resource Brokerage makes the enrollment process easier for you:
- Once you have the enrollment material completed to the best of your and your client’s ability, we will look over the paperwork and let you know if anything additional will be needed prior to submission.
- After submission of the enrollment material to the carrier, we provide the Welcome Letter to you and the Temp IDs to you upon request.
- Once the group is inforce and active with the carrier, we service the group with you. We are here for you every step of the way!
