The information needed for the quote is:

• Group name
• Group zip code
• Effective date
• Full name, gender, and date of birth for each enrollee

For 51+ Groups, we must request an underwritten proposal from the carrier. An underwritten proposal is based on both the average age of the census and the overall health of the group.

The information needed to request underwritten rates from BCBSIL is

• A completed version of the quoting census
• A completed version of the carrier specific RFP (Request for Proposal) form
• A copy of the group’s most current renewal available
• A copy of the group’s SBCs for their current plans
• A copy of the group’s large claims report
