For a new group BCBSIL enrollment, here is what we need:

• A completed and signed version of the BCBSIL enrollment packet.
• A copy of the group’s most recently filed quarterly wage & tax.
• If the owner is not listed on payroll, we’ll need a copy of the owner’s most recently filed tax document (Schedule C or K1).
• A completed version of the enrollment census or employee app/waiver for every full-time employee.

Note: Additional forms may be required as enrollment requirements vary from situation to situation.
